log in to your sistersonly account on the national website for Zeta Tau Alpha

click "submit MIS Form" at the bottom of the page

fill it out and submit it to our chapter!

scroll over to the "engage" tab and click "MIS Form"

click "+ create a reference for a potentional new member"

Our chapter requires a PNM to have one MIS form which is found on our website zetataualpha.org under the "Sisters Only" tab, look for an option to "Recommend a Member."
